This is for Mail Order, Telephone Order or Internet Merchants where 80% of the time (or more), the card is NOT present. Credit card information must be key entered into a terminal, online gateway or internet web page shopping cart system.

Complete the MOTO Online Application.

Print, Sign and Fax in your application.

Make sure you include the following supporting documentation with your faxed application:

• Blank Voided Check (must be pre-printed, no starter checks).
• Copy of Driver's License (lighten and enlarge before faxing).
• Copy of Business License or Sales Tax Permit.
• If you are currently accepting credit cards, please include 3 months processing statements for auto approval.
• Home-based businesses will also need to fax a sample ad showing your method of advertising.
