80% or more of your transactions MUST be swiped cards (card present). If this is not the case, please use your browser's back button and select the MOTO option.

Complete the RETAIL Online Application.

Print, Sign and Fax in your application.

Make sure you include the following supporting documentation with your faxed application:

• Blank Voided Check (must be pre-printed, no starter checks).
• Copy of Driver's License (lighten and enlarge before faxing).
• Copy of Business License or Sales Tax Permit.
• If you are currently accepting credit cards, please include 3 months processing statements for auto approval.
• Home-based businesses will also need to fax a sample ad showing your method of advertising.
